This episode began with a courier walking into an office building. He jumped into the elevator with another man going to the same floor. As thy were leaving the cournier noticed the older man had a bloody nose and he didn't look so hot. The man collapsed on the floor and the courier did CPR, after a few tries he said the man was dead. The 'dead' man's veins were all visible and he emitted a bloody mist from his mouth into the office air.
Olivia and Peter were the first on the scene to investigate this strange occurrence. They were questioning the witnesses when they noticed the courier man was missing. As Walter, Astrid, and Broyles approached the outside door they saw the courier trying to exit the building. Peter saw that the man had a bloody nose and he held the door shut. The courier's veins protruded like the previous case and he collapsed, emitting the bloody mist from his mouth. Everyone inside the building was quarantined, including Olivia and Peter.
The CDC arrived on scene to investigate. Walter barged into the CDC tent, got arrested, and had to be bailed out by Broyles so that he could get to his lab to start investigating the possible virus. Meanwhile, inside the building, Olivia and Peter noticed that the secretary who was closest with the courier was not feeling well so they made her comfortable on the floor and secluded her in a room. While they were discussing tactics, the secretary left the room and ran into a window and plummeted to her death from the 16th floor. While trying to help the secretary before jumping from the window, Peter slipped and fell into the first man's body and got his blood all over him. Peter washed it off, but both him and Olivia knew what would occur next.
Peter figured since he was already infected, he would search the body of the dead man to find a reason for coming to that building and possibly where he got infected. He found a car rental key in the man's pocket. A briefcase was found in the rental car containing a sample from deep inside the earth's crust and it also contained the virus that killed all life on earth many years ago. Walter and Astrid went to the lab to create a toxin screen to figure out who was infected. He explained to Astrid that the virus was trying to get loose from the contained environment to spread, causing the courier and secretary's attempt to escape the building.
Astrid and Walter took the toxin screen into the building to determine who was infected. Olivia tested negative, but Peter took her swab, without anyone looking, as his own. They were taking the first uninfected bunch back outside when a CDC member noticed Peter's bloody nose. He was once again trapped inside. Astrid found out from Olivia that Peter was infected. Walter confessed that he did not want to see Peter die again so he tried to think of a cure. He related it back to when the virus came about last time and determined that sulfur (originally from the volcanic eruption that wiped out the virus) could cure it. They went into the kitchen and used horseradish to create an experiment and it worked on the first dead man's blood. Walter called Olivia and told her the cure. They were running out of time downstairs with the infected trying to break out and they only had little time to make the cure before the building was blown up to eradicate the infection.
Olivia told Broyles to gas the building to give them more time. She volunteered herself to go inside and turn the airflow back on. She got to the parking deck when she was attacked by Peter, he stole her gun and ran off to use it to try and escape. Olivia got to the electric panel and turned on the airflow just in time for the Army to enter the building with everyone passed out from the gas. All of the infected were given the cure and were recovered. Peter thanked Olivia for being sane when he was not.
While leaving the scene, Astrid asked Walter what he meant about Peter dying again, and he refused to answer her.

It was more tense than other ones because the beloved Peter was involved and a lot of the resolutions came down to the last minutes in the episode. I also appreciated the fact that now someone else knows that Peter had died once before which ties into the 'other universe' theory of the show.
